The CardioSmart Hypertension Management Program is designed to provide guidance and tools to help you manage your blood pressure and improve your overall well-being. The main feature of this program is the online CardioSmart Blood Pressure Tracking Tool (powered by PeopleChart). The tool prompts you to enter your blood pressure readings, heart rate, and weight on a regular basis. The information you enter will also be made available to your health care provider(s) so that they can view the progress you make in real time.
