AMA Weigh What Matters
The AMA Weigh What Matters app makes it easy to work with your physician to establish weight, healthy eating, and physical activity goals, and track your progress toward achieving them. It also calculates your BMI score, and provides a mechanism to view progress reports and email them to your physician.The app is part of Weigh What Matters, a family obesity prevention program developed by the American Medical Association, which encourages patients to work with their physicians to maintain a healthy weight, eat better, and increase physical activity. Weigh What Matters is part of the AMA Healthier Life Steps™ initiative. This app is available for the Apple iPhone®, iPad® and iPod Touch® on the AMA iTunes store, and for Android devices.

Health Butler
Health Butler is a great resource that compiles the most up-to-date and well-researched preventive healthcare information available and presents you the results in an organized, cohesive format.

Pre Existing Condition Insurance Plan
As a result of the federal Affordable Care Act of 2010, California has a contract with the federal Department of Health and Human Services to establish a federally-funded high risk pool program to provide health coverage for eligible individuals. The program will last until December 31, 2013 when the national health reform is set to begin. After that date, there will no longer be a need for high risk pools because federal rules will not allow insurers to reject persons with pre-existing conditions or charge them higher rates than those without such conditions.

AMA's Healthier Life Steps:
Four key health behaviors, poor diet, physical inactivity, tobacco use, and excessive or risky use of alcohol, are significantly related to heart disease, diabetes, cancer, and other conditions. This program provides a tool-kit to help you partner with your physician to improve these key health behaviors. The tool-kit includes a self-assessment questionnaire, action plans, and progress tracking calendars, as well as links to other resources. Talk to your physician about your plans to make sure you are on the right track and getting the support you need to make these changes in a healthy way.

California Hypertension Management Program:
The CardioSmart Hypertension Management Program is designed to provide guidance and tools to help you manage your blood pressure and improve your overall well-being. The main feature of this program is the online CardioSmart Blood Pressure Tracking Tool (powered by PeopleChart). The tool prompts you to enter your blood pressure readings, heart rate, and weight on a regular basis. The information you enter will also be made available to your health care provider(s) so that they can view the progress you make in real time.

Personal Nutrition Planner:
Go to the meal planning website Meals Matter and try the Personal Nutrition Planner. After a quick assessment, you will learn your Body Mass Index (BMI), total calorie needs and food serving suggestions to match your personal health risks and lifestyle preferences. The program includes a planning section and follow up email that promotes behavior change.
